Totally drug-resistant tuberculosis strains: evidence of adaptation at the cellular level.

چکیده
In the Eastern Mediterranean Region (EMRO), there are no accurate estimates of numbers of patients with multidrugresistant (MDR)/extensively drug-resistant (XDR) tuberculosis (TB). In 2006, we documented the existence and transmission of XDR-TB among patients with MDR-TB [1]. In a further study, we isolated more dangerous forms of bacilli, named totally drug-resistant (TDR) [2]. This group of strains showed in vitro resistance to all firstand second-line drugs tested (i.e. aminoglycosides, cyclic polypeptides, fluoroquinolones, thioamides, serine analogues and salicylic acid derivatives). TDRTB patients remained smearand culture-positive after 18 months’ median treatment with second-line drugs. Even changing the treatment to co-amoxyclav (625 mg per 8 h) or clarithromycin (1,000 mg?day), along with a high dose of isoniazid (15 mg?kg) led to no improvement [2]. In the present study, transmission electron microscopy (TEM) was used to evaluate the differences between such deadly bacilli and MDR or susceptible ones at the ultrastructural level. Furthermore, to evaluate the genotyping patterns, the strains were subjected to spoligotyping and variable number of tandem repeats (VNTR) analysis.

Evaluation of Gene Mutations Involved in Drug Resistance in Mycobacterium Tuberculosis Strains Derived from Tuberculosis Patients in Mazandaran, Iran, 2013
Drug resistance (especially multiple drug resistance) in Mycobacterium tuberculosis makes global concerns in treatment and control of tuberculosis. Rapid diagnosis of drug resistant strains of the bacteria has vital importance in the prognosis of the disease. The aim of this study was to identify the mutations responsible for drug resistance in Mycobacterium tuberculosis strains derived from pa...
Background and Objective: Drug resistance to tuberculosis is continuously increasing and is a significant threat to tuberculosis control programs because afew effective drugs are present against Mycobacterium tuberculosis. Although isoniazid (INH) is the most effective drug against tuberculosis, resistance to this drug also develops readily. Mutations in katG, specially the Ser315Thr substituti...
متن کاملDetection of Isoniazid and Rifampin Resistant Strain of Mycobacterium Tuberculosis Isolated from patients in Golestan province (North of Iran)
Abstract Background and objectives: With almost nine million new cases each year, tuberculosis is still one of the most Life-threatening diseases in the World. Distribution of drug resistant strains of M.tuberculosis has a lot of importance. This research was carried out to determine the frequency of drug resistance of M. tuberculosis in strains isolated in Golestan province. Material and...
متن کاملPrevalence ofMultidrug Resistant Mycobacterium tuberculosis by Mycobacteria growth indicator tube in Golestan province, North of Iran
Abstract Background and objectives: Identification and monitoring of multidrug-resistant Mycobacterium tuberculosis strains (MDR) is highlighted by the high risk of their spreading in different areas. Prevalence of these strains was evaluated in Golestan province in northeast of Iran. Material and Methods: Drug susceptibility testing to Isoniazid and rifampin was carried out for 148 clinical sa...
متن کاملPoint-Mutations in embB306 Gene and Their Association with Resistance to Ethambutol in Mycobacterium tuberculosis in Clinical Isolates
Background & Objective: Mutations in embB306 gene and their association with resistance to ethambutol (EMB) in Mycobacterium tuberculosis (M. tuberculosis) have not been fully investigated. The aim of this study was to investigate the point-mutations in emb306B gene and their association with resistance to EMB in M. tuberculosis. Materials & Methods: This case (M. tuberculosis resistant to EMB...
